 We’re excited that you’re planning on joining us at the 129th Annual Grand Session of Eureka Grand Chapter Prince Hall Order Eastern Star. This event will take place at the Hyatt Regency Rochester, 125 E Main St, Rochester, NY 14604.  The health and safety of our Officers, Members and Guest are of the utmost importance to us. So we’ve taken steps to ensure that your experience is both safe and memorable.

Before you can register for the events and purchase tickets, it is required that you agree to the following Waiver.

Attendee Medical Liability Waiver & Assumption of Risk

• I voluntarily seek admission to the 129th Annual Grand Communication. Activities for this event will take place on Sunday, May 31, 2026, to Tuesday, June 2, 2026, at the Hyatt Regency Rochester, 125 E Main St, Rochester, NY 14604.

• Therefore, I, on behalf of myself, my heirs, executors, administrators, and personal representatives, hereby release, waive, discharge, and agree to hold harmless Eureka Grand Chapter, Prince Hall Order of Eastern Star, State of New York, and the Most Worshipful Prince Hall Grand Lodge of the State of New York, together with their officers, members, agents, representatives, volunteers, and employees, from any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses of any kind, whether known or unknown, arising out of or related to my attendance at and participation in the conference and any associated activities.

  This release includes, but is not limited to, claims for bodily injury, illness, death, property damage, loss, or any other harm that may occur while attending or participating in conference-related events, whether caused by negligence or otherwise, to the fullest extent permitted by law.

  I acknowledge that I am voluntarily participating in this event and assume full responsibility for any risk of injury or loss associated with my attendance.
